HAS Foundation: Grant Application Interest Form

The HAS Foundation Financial Assistance Program Grant offers up to $2,000 in support for individuals living with ALS, Alzheimer’s, dementia, or brain injuries. This grant is designed to help reduce financial obstacles that may limit access to specialized clinical care for these conditions. Eligible expenses may include transportation, lodging, lost wages, copays, and deductibles directly related to clinic visits for brain-related conditions.
